USCG:ISM滯留,重發(fā)!

2019年1月份,USCG共滯留船舶8艘,每艘船的滯留各有特別,如沒特別事項,從今日起,一個個的分享給大家。文末一張小照片女警官介意,換位不介意的,重發(fā)一次,請各位諒解。

八艘滯留船舶

今天分享的是因兩條缺陷被滯留的,一條體系缺陷,一條一般缺陷。

這艘船舶:PREMIUM DO BRASIL(9242089),利比里亞船籍,DNV-GL船級社,1月8日被滯留。

管理公司及owner如下

第一項滯留缺陷:

015109 : Maintenance of the ship and equipment

Objective evidence discovered during an expanded ISM exam revealed the following non-conformities. These discrepancies included with the other material deficiencies are evidence that crew/company are not meeting the SMS requirements. Recommend an external audit

ISM擴(kuò)大檢查中發(fā)現(xiàn)一系列不符合項目,這些不符合項目說明SMS體系沒有有效運行,建議進(jìn)行外審。這些技術(shù)性缺陷是:

1.PSCO observed soft patches in the engine room on the auxiliary salt water line andthe #4 main engine cylinder fuel pump drain line. The patches were not approved by the vessel's class nor were there any corrective actions taken IAW the vessel’s SMS.

主機(jī)4號缸高壓油泵泄放管和輔機(jī)海水冷卻管線泄露,進(jìn)行了臨時修理,但是沒有經(jīng)過船級社的檢驗認(rèn)證,或者按照公司SMS體系的要求進(jìn)行報告處理。對于臨時修理,USCG是不接受的,請參考往期推送自修,也會被滯留,一條就夠了!

2.PSCO also observed a flexible hose installed in the drain line from fuel oil hot box which was not of approved material, nor were there any corrective action taken IAW the vessel’s SMS

燃油泄放管路上非法連接了一段軟管,該軟管未經(jīng)認(rèn)可。

3.The main engine was observed to have fuel soaked lagging, piping and housing in the vicinity of the fuel pumps. Additionally, in the automatic fuel filter area, fuel spray was apparent and observed on the ceiling, covering the piping and the lagging, and inside the electrical control boxes for the automatic fuel filter system. Being a reoccurring issue, a plastic curtain was installed around the system to shield the rest of the engine room from the fuel spray. 主機(jī)及燃油管線附件布滿燃油和棉紗,估計是漏油所致,為了防止燃油飛濺到主機(jī),安裝了一個塑料擋板。

4.PSCO observed an approximately 15 inch crack in the crank case housing of the main engine. PSCO observed documentation showing that the crack had been reported on January 18, 2018, but no non-conformity report have been filed or corrective actions have been implemented. 曲軸箱有一個15英寸的裂紋,根據(jù)記錄顯示,2018年1月18日,已經(jīng)將這一缺陷報告給公司,但是沒采取任何整改措施。

5. Crew members were assigned a number and not all crew members knew their assigned number or duties. Additionally, muster list does not instruct fire fighting team to carry radios as per 74SOLAS (14 Cons). Furthermore, PSCO discovered vessel was not conducting enclosed space entry and 

rescue drills as required by 74SOLAS (14 Cons). Vessel was unable to produce sufficient procedures or checklist for training an ensuring personnel knew what to do in the event of an emergency。各船員不清楚自己的職責(zé),應(yīng)急部署表沒有安排消防隊攜帶雙向?qū)χv設(shè)備,船上沒按照要求開展進(jìn)入封閉艙室和救生演習(xí)。

6.All items of rescue boat equipment shall be secured within the rescue boat. PSCO observed all required equipment was stored in a separate locker on deck away from the rescue boat.救助艇內(nèi)的設(shè)備及附件需要固定在救助艇內(nèi),但該輪的附件被鎖在甲板上的一個儲藏室里。

毋庸置疑,上面六個缺陷可以推算出該輪的管理狀態(tài)很差。PSCO根據(jù)上面6項技術(shù)性缺陷推斷船舶SMS體系運行失效,需要進(jìn)行附加審核。長期閱讀PSCReady文章的人都知道,現(xiàn)在檢查并發(fā)現(xiàn)ISM缺陷是PSCO屆的一個趨勢!從這則報告看USCG完全掌握這項技能!

傳統(tǒng)印象,總有人說USCG是大兵,不懂技術(shù),從這則報告,羅列的缺陷和ISM缺陷的判斷,這位PSCO的水平還是蠻高的,下手穩(wěn)準(zhǔn)狠,是個角色!ISM滯留船確實管用!

首先是硬件,主機(jī)漏油,海水管損壞,曲軸箱裂紋,救生設(shè)備管理不當(dāng),隨意存放

其次是軟件,人員不熟悉各自職責(zé),演習(xí)沒按照要求開展。

推斷清晰!

出于好奇,我查看了下這艘船,還算蠻大的,205米,2002年建造,管理成這個樣子,實屬不易,哪里的管理團(tuán)隊,會這么差呢?

第二項滯留缺陷是:

07126 - Oil accumulation in engine room 

PSCO observed an overall hazardous condition of the engine room. PSCO observed the main engine to have fuel soaked, lagging, piping, and housing in the vicinity of the main engine fuel pumps. Additionally, in the automatic fuel filter area, a history of fuel spray was apparent and observed on the ceiling, covering the piping and lagging, and inside the electrical control boxes for the automatic fuel filter system. Being a reoccurring issue, a plastic curtain was placed around the system.

從主機(jī)燃油系統(tǒng)周圍的情況推斷,該系統(tǒng)曾發(fā)生過漏泄,未防止再次發(fā)生,船員用塑料擋板罩住,主機(jī)周圍漏油,布滿油布/棉紗。存在很大的安全隱患。

毫無疑問,這種船理應(yīng)滯留,理應(yīng)附加審核。造成這種情況的原因自然是多方面的

  1. 公司支持不足。18年1月報告的不符合項目,公司拖了一年依然不給解決,船員難為無米之炊;

  2. 船員業(yè)務(wù)素質(zhì)差,不夠職業(yè)。岸基支持不夠,船員可以采取繼續(xù)上報,或者向PSCO投訴的方式解決這一問題,船員投訴公司支持不夠,會直接導(dǎo)致滯留,而這艘船舶的船員,不熟悉自己的崗位職責(zé),竟然連演習(xí)都不按照計劃開展,這是搞哪樣?一旦出現(xiàn)緊急情況,難道GOD來幫你么?

  3. money不足,公司縮減投入,請不起敬業(yè)船員,供不起足夠備件,但是請各位記住,安全是第一位的,安全的錢不能省,也不能忽視,提醒各位對自己負(fù)責(zé)。提醒各位遠(yuǎn)離這種不負(fù)責(zé)任的管理團(tuán)隊!
