頸內(nèi)動(dòng)脈解剖（1）八種分段方式詳解

頸內(nèi)動(dòng)脈（internal carotid artery，ICA）是自頸總動(dòng)脈（common carotid artery，CCA）分叉處發(fā)出，最終分為大腦前動(dòng)脈和大腦中動(dòng)脈兩個(gè)末端分支。主要為大腦前循環(huán)供血。

大體解剖結(jié)構(gòu)

頸總動(dòng)脈（common carotid artery，CCA）左側(cè)起源于主動(dòng)脈弓，右側(cè)起自頭臂干（又稱無(wú)名動(dòng)脈）。CCA分叉形成ICA和頸外動(dòng)脈（external carotid artery，ECA）。ICA的起始部有一個(gè)擴(kuò)張稱為頸動(dòng)脈球或竇（carotid bulb or sinus），是頸動(dòng)脈體（carotid body）的位置。頸動(dòng)脈竇作為血壓變化的壓力感受器，而頸動(dòng)脈體則作為檢測(cè)酸堿紊亂的化學(xué)感受器。

多數(shù)情況下頸總動(dòng)脈分叉發(fā)生在C3-C5椎體水平，或舌骨和甲狀軟骨上緣之間水平。雙側(cè)分叉水平可能不對(duì)稱。血管發(fā)育變異等不在此展開(kāi)討論。

分段

基于外科手術(shù)和介入治療的考慮，許多學(xué)者提出了ICA的分段系統(tǒng)。按提出的時(shí)間順序排列（名字是我瞎起的，別認(rèn)真記）：

1.1938年，F(xiàn)ischer逆血流五段法：The positional deviations of the anterior cerebral artery in angiograms [Die Lageabweichungen der vorderen hirnarterie im crefassbild] Zentralbl Neurochir, 3 (1938), pp. 300-313.

目前應(yīng)用最廣泛的是Bouthillier七段法，其他的了解下就好。
